NEW CLIENT/ NEW SERVICE CHECK-IN At National Laser Institute Medical Spa, we are committed to providing a relaxing and enjoyable experience. We also want to give each one of our clients the specialized time and attention he or she deserves during treatment. If you are a new client, please arrive 15 minutes early. If you are an existing client who has scheduled a new treatment for the first time, please arrive 15 minutes early as well. This will allow you to fill out the necessary paperwork, including a medical history and consultation form. We thank you for your cooperation.
CANCELATIONS Appointments at National Laser Institute Medical Spa are scheduled in advance, and we do not double-book. We require a 48-hour notice to cancel or reschedule an appointment. We also require a credit card to be kept on file, and in the event that you miss a scheduled appointment, or do not cancel your appointment 48 hours prior, your card will be charged for the full service price. If you arrive more than ten minutes late for your appointment, it will be considered a “no show”. At that time, your card will be charged for the full service price. Tattoo Removal Clients will be charged $100 for a no show/less than 48 hours cancellation notice – Even if their appointment charge is less.
AGE REQUIREMENTS We do perform treatments on clients under the age of 18, but a parent or guardian must co-sign all consent forms and be present on the first day of treatment. Clients 16 years of age or younger must have a parent or guardian present at all times.
